Navy bootcamp questionnaire

hi everyone, Navyprincess here. I used to be on here a lot when I was joining the navy and since then ive been offering wisdom but now I need your help. my brother leaves for the navy in 12 days and im trying to think of a funny questionnaire I could send him that would make him laugh. I'm blanking since its been about 2 years since ive been there.

Any ideas?????
